Key Responsibilities:

• Review and evaluate medication orders for drug therapy appropriateness, including dose, form, regimen, duplication, and potential interactions
• Provide specialized pharmacokinetic and anticoagulation dosing services
• Engage in clinical pharmaceutical care, including medication reconciliation and discharge medication education
• Oversee pharmaceutical compounding processes in compliance with USP 797 and USP 800 regulatory guidelines
• Effectively utilize EPIC healthcare IT systems for medication management and documentation

Qualifications:

• Licensed pharmacist with current Wisconsin license or eligible for temporary Wisconsin licensure
• Minimum of 1 year hospital pharmacy experience
• Strong clinical skills in medication management and patient care
• Familiarity with USP compounding standards and EPIC system proficiency preferred
