Job Responsibilities:

• Assess, plan, implement, and evaluate patient-centered care plans that address social, emotional, and psychological needs of patients

• Develop and maintain strong relationships with patients, families, and healthcare teams to ensure comprehensive care

• Provide access to individual, group, and family therapy sessions to address mental health concerns, coping strategies, and life skills

• Conduct evidence-based interventions to address patient-specific needs, including crisis intervention, case management, and community resource coordination

• Collaborate with healthcare providers to develop patient-centered care plans and improve healthcare outcomes

• Maintain accurate and timely documentation of patient services and progress

Requirements:

• Current licensure as a Licensed Social Worker (LSW) in the state of Minnesota

• Minimum of 3 years of experience in social work, preferably in a long-term healthcare setting

• Strong communication, interpersonal, and organizational skills

• Ability to work effectively in a fast-paced, dynamic environment
