We are seeking a dedicated and clinically skilled Licensed Social Worker to join our interdisciplinary team at Kadima Rehabilitation & Nursing at Mt. Lebanon. As a valued member of our team, you will play a crucial role in providing high-quality care and support to our residents and their families.
About the Role:
As a Licensed Social Worker, you will be responsible for assessing the social and emotional needs of our residents and developing individualized care plans to address their unique needs.
You will work closely with our interdisciplinary team, including physicians, nurses, and therapists, to ensure comprehensive care and support for our residents.
You will provide counseling, support, and education to our residents and their families to promote emotional well-being, independence, and overall quality of life.
You will maintain accurate and timely records of resident assessments, care plans, and progress notes.
Responsibilities:
Conduct initial assessments of new residents to identify social and emotional needs.
Develop and implement individualized care plans to address resident needs.
Provide counseling, support, and education to residents and their families.
Participate in interdisciplinary team meetings to discuss resident care and progress.
Maintain accurate and timely records of resident assessments, care plans, and progress notes.
Identify and report potential social and emotional concerns or issues affecting resident care.
