Responsibilities:
Provide compassionate and individualized social work services to residents, family members, and caregivers
Conduct comprehensive initial assessments and develop personalized care plans that address the physical, emotional, and social needs of our residents
Collaborate with interdisciplinary teams to ensure seamless care coordination and high-quality care delivery
Facilitate resident and family counseling, education, and support to promote independence, self-advocacy, and overall well-being
Develop and lead groups, workshops, and programs that foster social connections, community building, and personal growth
Maintain accurate and detailed records, including assessments, care plans, and progress notes
Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care delivery
Requirements:
Licensure: Current and valid LSW or LCSW licensure in the state of Michigan
A strong commitment to delivering compassionate, patient-centered care with a focus on holistic well-being
Excellent communication, interpersonal, and organizational skills
Ability to work effectively in a fast-paced environment with multiple priorities and deadlines
Strong attention to detail and ability to maintain accurate records
