Home Health RN - NW Side of San Antonio

American Medical Home Health Services is seeking a dependable and experienced Registered Nurse (RN) to provide home health visits in Northwest San Antonio and surrounding areas.

Position Details

• PRN/per-visit position
• Flexible scheduling
• Competitive per-visit rates
• Patient assignments primarily in Northwest San Antonio
• Electronic documentation through WellSky/Kinnser
• NestMed AI available to assist with clinical documentation

Responsibilities

• Complete comprehensive patient assessments
• Perform skilled nursing visits according to the plan of care
• Complete Start of Care, Resumption of Care, Recertification, and Discharge assessments, Supervisory, Routine visits as assigned
• Communicate changes in the patient’s condition to physicians and the clinical team
• Provide patient and caregiver education
• Submit accurate and timely clinical documentation
• Coordinate care with therapists, providers, and other members of the interdisciplinary team

Qualifications

• Current and unrestricted Texas RN license
• Minimum of one year of nursing experience
• Home health and OASIS experience strongly preferred
• Current CPR certification
• Reliable transportation, valid driver’s license, and automobile insurance
• Ability to manage visits independently and complete documentation promptly
