American Medical Home Health & Hospice has a long-standing history of providing compassionate, patient‑centered hospice care across Texas. As we continue to serve the San Antonio community, we are seeking a PHC Coordinator who can join our team to work directly in improving lives of families in the San Antonio community.

We’re looking for a highly organized, compassionate, and driven PHC Coordinator to lead day-to-day operations of our Primary Home Care program. If you thrive in a fast-paced environment and enjoy balancing people, scheduling, and problem-solving—this role is for you.


As a PHC Coordinator, you are the heart of our operations, ensuring clients receive consistent, high-quality care while supporting and guiding our caregiving team.

Care Coordination & Scheduling

  • Manage daily schedules for caregivers, ensuring full coverage of all client needs
  • Match caregivers with clients based on skills, personality, and availability

Client & Family Support

  • Serve as the primary point of contact for clients and families
  • Address concerns, changes in condition, and service requests promptly

Team Collaboration

  • Work closely with nursing and clinical staff to ensure seamless care delivery
  • Support onboarding and scheduling setup for new caregivers

Compliance & Operations

  • Ensure accurate documentation, service logs, and timesheets for billing
  • Monitor EVV, attendance, and service quality standards
  • Maintain full compliance with Texas HHS and Medicaid PHC regulations

Problem-Solving & Crisis Management

  • Respond quickly to call-offs, emergencies, and staffing gaps
  • Keep operations running smoothly under pressure


Experience:

  • At least 1 year of experience in home care, home health, or PHC/PAS programs
  • Strong multitasking, scheduling, and organizational skills
  • Excellent communication and customer service abilities
  • Ability to stay calm and effective in a fast-paced environment
  • Proficiency with EMR systems and scheduling software


Preferred Qualifications

  • Bilingual (English/Spanish) – highly valued in the San Antonio community
  • Experience with Texas Medicaid programs (PHC, PAS, EVV compliance)
  • Healthcare administration or related education
