As a Home Health RN with Orion Home Health, you will play a vital role in providing high-quality, individualized care to patients in their homes, promoting independence, comfort, and well-being. Responsibilities:
Conduct initial patient assessments, develop personalized care plans, and provide skilled nursing care to patients in their homes
Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to ensure comprehensive care
Administer medications, perform wound care, and provide education to patients and their caregivers
Monitor patients' progress, report changes, and make recommendations for adjustments to care plans
Maintain accurate and detailed records of patient interactions, treatments, and progress
Requirements:
Current registration with the Illinois Board of Nursing (IBON) as a Registered Nurse (RN)
Valid Driver's License (DL) and reliable transportation
Minimum 0 years of experience in home health or a related field (new graduates welcome to apply!)
