As a Mental Health Rehabilitation Worker, you will play a vital role in our multidisciplinary team, working closely with licensed professionals to deliver high-quality rehabilitation services to our clients. Responsibilities:

• Provide direct care and support to clients in various settings, including group homes, outpatient facilities, and community-based programs
• Assist with daily living activities, such as personal care, meal preparation, and housekeeping
• Facilitate social and recreational activities to promote client engagement and well-being
• Maintain accurate and timely records of client progress and services provided
• Collaborate with treatment teams to develop and implement individualized treatment plans
• Provide crisis intervention and stabilization services as needed
