Position Summary

Triple Ace Home Health Agency is seeking a dependable and compassionate Physical Therapist Assistant (PTA) for a PRN position providing skilled therapy services to patients in their homes. The PTA will work under the direction and supervision of a licensed Physical Therapist to help patients improve mobility, strength, balance, and functional independence.

Responsibilities

• Provide skilled physical therapy interventions according to the Physical Therapist's plan of care.

• Perform therapeutic exercises, gait training, balance activities, transfer training, and mobility interventions.

• Monitor patient response to treatment and report changes to the supervising Physical Therapist.

• Educate patients and caregivers on exercises, safety, mobility, and fall prevention.

• Complete all required clinical documentation accurately and timely.

• Communicate with the Physical Therapist and interdisciplinary care team regarding patient progress.

• Follow agency policies and Medicare, Medicaid, and applicable regulatory requirements.

Qualifications

• Current, unrestricted Physical Therapist Assistant (PTA) license in the state of practice.

• Graduate of an accredited PTA program.

• Current CPR certification.

• Home health experience preferred.

• Strong communication and documentation skills.

• Reliable transportation and ability to travel to patients' homes.

• Ability to work independently and maintain professional standards.

Compensation & Employment

• $35 per completed visit

• PRN W-2 position

• $100 one-time bonus after 90 days of employment, subject to satisfactory performance and completion of the 90-day evaluation.
