Spring River Rehabilitation & Care Center is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our clinical team.
As an LPN, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary care team to promote safe, high-quality resident outcomes. You will also provide direction and support to CNAs and other nursing team members while helping ensure individualized care plans are followed.
This position is ideal for a nurse who values hands-on resident care, teamwork, and the opportunity to build meaningful relationships with residents and their families.
Key ResponsibilitiesProvide direct nursing care in accordance with individualized care plans, physician/provider orders, and established clinical standards
Monitor resident conditions and promptly communicate significant changes to the appropriate nurse, provider, and members of the care team
Administer medications, treatments, and therapeutic interventions safely and accurately within the LPN scope of practice
Obtain and monitor vital signs and other clinical information necessary to evaluate resident health
Collaborate with physicians/providers, RNs, CNAs, therapy, and other interdisciplinary team members to coordinate resident care
Provide education and guidance to residents and families regarding medications, treatments, and care needs within appropriate scope
Supervise, delegate, and support CNAs and other nursing personnel in the delivery of resident care
Ensure individualized care plans and nursing interventions are implemented consistently
Maintain accurate, complete, and timely documentation within the electronic medical record (EMR)
Recognize and respond appropriately to changes in resident condition and urgent or emergency situations
Participate in infection prevention, resident safety, quality assurance, and performance improvement initiatives
Promote resident dignity, privacy, independence, comfort, and quality of life
Maintain resident confidentiality and comply with applicable professional, facility, state, and federal requirements
Graduate of an accredited practical nursing program
Current, active Licensed Practical Nurse (LPN) license in good standing in the State of New Mexico
Current CPR or Basic Life Support (BLS) certification
Previous skilled nursing, long-term care, rehabilitation, or geriatric nursing experience preferred
Strong nursing assessment, observation, and clinical decision-making skills
Ability to provide compassionate, person-centered care to residents with varying physical, cognitive, and clinical needs
Strong communication and interpersonal skills
Ability to effectively supervise, delegate, and support nursing assistants
Ability to prioritize responsibilities and adapt to changing resident needs in a fast-paced clinical environment
Strong attention to detail and commitment to accurate clinical documentation
