Full-Time Home Health LPN – Tempe, South Scottsdale, South Phoenix & Ahwatukee

Dependable Health Services is seeking an experienced Licensed Practical Nurse (LPN) to join our Home Health team full-time. We are looking for a dependable, organized nurse who is comfortable working independently in the field and understands the pace, documentation, and coordination required in home health.

What You'll Do

As a Home Health LPN, you will provide skilled nursing care to patients in their homes under the direction of the RN and physician's plan of care. Responsibilities include:

  • Provide skilled nursing visits throughout your assigned territory

  • Perform assessments and monitor changes in patient condition

  • Administer medications and treatments as ordered

  • Provide wound care and other skilled nursing services within LPN scope of practice

  • Educate patients and caregivers regarding medications, treatments, and disease management

  • Communicate changes in condition promptly to the RN and appropriate members of the care team

  • Complete accurate and timely clinical documentation

  • Coordinate with RNs, therapists, physicians, and other members of the interdisciplinary team

  • Maintain compliance with agency policies and home health regulations

What We're Looking For
  • Current Arizona LPN license in good standing

  • Previous home health experience is required

  • Kinnser/WellSky experience is strongly preferred

  • Strong clinical assessment and documentation skills

  • Excellent communication and time-management skills

  • Ability to independently manage a field schedule

  • Reliable transportation and willingness to travel throughout the assigned service area

  • Current CPR certification

  • Ability to meet all employment and compliance requirements

Tempe | South Scottsdale | South Phoenix | Ahwatukee
