About CHDC:
CHDC (Community Health and Dental Care, Inc.) is a quickly growing Community Health Center in Pennsylvania with locations in Pottstown, Barto, and Boyertown, providing care for all patients in our service area, regardless of insurance status or ability to pay. Our staff put the care and well-being of our patients at the forefront of everything we do at CHDC.
Position Summary:
Provide clinical behavioral health services in a manner that upholds the health center’s mission and quality standards, and results in improved patient health outcomes and satisfaction, as well as strong operational and fiscal capacity of the health center.
Role Qualifications:
Education: Minimum- Master’s degree in social work or related field
Experience: Minimum -2 years’ experience as a therapist; current license to practice in PA as a Licensed Professional Counselor; Competency in providing cognitive and behavioral interventions to patients with behavioral health/substance abuse issues in a primary care setting
Position Responsibilities:

Essential Skills & Experience:
• Consult with PCPs to enhance understanding of the patient, provide decision support for treatment planning and assist in the implantation and monitoring of bio-psychosocial treatment plans.
• Individual, couple, and/or family consultation with patients, including:
• Functional and strength-based assessment and diagnosis
• Psychoeducation for patients and their support systems
• Medication adherence counseling and disease self -management counseling
• Motivational Interviewing/Problem solving cognitive intervention.
• Self-Care Plan development and skills training
• Substance use/abuse evaluation and harm reduction strategies
•Charting as required includes assessments, progress notes, and billing.

• Manual Dexterity Required: Must be able to manipulate small electronic parts and controls.

• This position requires compliance with Health Center’s written standards, including its Standards of Conduct and policies and procedures (“Written Standards”). Such compliance will be an element considered as part of the employee’s regular performance evaluation.

Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.
