Job Description
This position is responsible for providing quality nursing oversight for individuals receiving Community Living Services at North Shore location with on call duties at South Shore locations. This position is responsible for executing CLS consumer’s comprehensive plan of care. Quality medical oversight includes, but is not limited to, reviewing medical evaluations and results, consulting with medical professionals, observing staff, reviewing medication administration and performing related duties as assigned to ensure that quality outcomes are achieve
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Complete annual and quarterly notes to each consumer.
- Review results from laboratories, medical evaluation and physician notes and records recommendations in consumer record book.
- Consult with physicians to provide accurate and complete information concerning current health and medical histories of consumers.
- Preparing reconciliation to the MARS.
- Answering calls from staff if there is an incident.
- Review medication administration records on a scheduled basis.
- Provided in-service training to staff as appropriate.
- Participate in consumer interdisciplinary team meetings.
- Observes staff performing health care procedures and provides evaluation of their performance.
- Provide 24 hour on-call coverage as needed.
- Evacuate with Community Living Services consumers during hurricane.
- Travel between North Shore and South Shore when needed.
- Perform related duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:
This position requires knowledge, skills and abilities equivalent to:
- Must have graduated from an accredited Nursing program and be licensed by the State of Louisiana as a Licensed Practical Nurse.
- Required to have at least two years of nursing experience.
- This position requires the incumbent to be licensed as a Practical Nurse and to be in good standing with the Nursing Board of Louisiana.
- Must have a valid Louisiana Driver’s license and be insurable under the VOASELA, Inc.’s liability insurance.
- Must be able to pass a criminal history investigation
- Able to obtain Cardio Pulmonary Resuscitation, First Aid certifications.
PHYSICAL REQUIREMENTS:
Physical requirements include sitting, standing, climbing, pushing, bending, squatting, lifting, and pulling. Maybe required to assist in moving consumers weighing up to 200lbs. Other physical requirements include:
- The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.
- Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
- Operating office equipment requiring continuous or repetitive hand/arm movements.
- Must be able to drive a vehicle to multiple group homes per day, or as required, in varying weather conditions and temperatures.
- Work is performed indoors and outdoors.
