We are seeking a compassionate and skilled LPN to join our team at Lamar Estates, a premier Assisted Living Facilities in Lamar, Colorado. As an LPN, you will play a vital role in providing exceptional patient care and support to our residents. With a salary range of $31 - $42 HOURLY, this is an exciting opportunity to make a difference in the lives of others.

Responsibilities:

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities

• Develop and implement individualized care plans in collaboration with the healthcare team

• Conduct regular assessments to identify changes in residents' conditions and report findings to the RN or physician

• Maintain accurate and timely records of patient care and treatment

• Participate in in-service training and educational programs to stay current with industry developments and best practices

• Collaborate with the interdisciplinary team to ensure seamless care delivery and resident satisfaction
