Creekside Care Center of Gahanna is seeking an energetic and compassionate PRN Physical Therapist (PT) to join our in-house rehabilitation team!

What You’ll Do- Evaluate and treat residents in a SNF/LTC setting
- Develop individualized plans of care based on resident needs and goals
- Provide skilled therapeutic interventions to improve mobility, strength, balance, and function
- Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
- Educate residents, families, and caregivers
- Complete timely and accurate documentation
- Utilize Net Health and PointClickCare (PCC)
- Current Ohio Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Strong clinical and communication skills
- Dependable, positive, and team-oriented
- SNF/LTC experience is a plus, but we welcome PTs interested in growing their experience in this setting
