We are seeking a dedicated and compassionate Licensed Physical Therapist to join our team of healthcare professionals at our Assisted Living Facilities in Lamar, CO. As a Physical Therapist, you will play a vital role in helping our residents regain independence and achieve their highest level of physical function.
As a Physical Therapist, your primary responsibilities will include:
• Evaluating and treating residents with various physical limitations and disabilities
• Developing and implementing personalized treatment plans to achieve optimal outcomes
• Conducting therapy sessions in a safe and supportive environment
• Collaborating with other healthcare professionals to ensure comprehensive care
• Maintaining accurate records and documenting resident progress
• Physical Therapist duties may also include facilitating resident transfers, using assistive devices, and performing other physical activities to support resident care

Requirements:
• Bachelor's degree in physical therapy
