What You’ll Do:

• Provide PTA services/establish plans of care, management
• Treat patients in SNF and/or Outpatient settings as appropriate
• Collaborate closely with an interdisciplinary team
• Deliver individualized, meaningful patient care
• Complete documentation efficiently and accurately

What We’re Looking For:

• IL Licensed Physical Therapist Assistant (PTA)
• Strong communication and teamwork skills
