Heather Hill Nursing Center is excited to offer an opportunity for a Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in New Port Richey, FL. This role is ideal for a clinician who values patient-centered care and thrives in a collaborative skilled nursing environment.

Our in-house therapy model supports consistent care, strong interdisciplinary collaboration, and individualized treatment plans focused on improving residents’ independence and overall quality of life.

Position Overview

As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services to improve functional abilities, safety, and participation in daily activities.

Key Responsibilities

• Evaluate residents to identify functional impairments and therapy needs

• Develop and implement individualized treatment plans focused on ADLs and IADLs

• Monitor resident progress and adjust treatment plans as needed

• Recommend adaptive equipment and environmental modifications

• Educate residents, families, and caregivers on therapeutic techniques

• Maintain accurate and timely documentation per facility and regulatory guidelines

• Collaborate with the interdisciplinary team to ensure optimal outcomes

• Maintain compliance with all facility, state, and federal regulations

Qualifications

Active Florida Occupational Therapist (OT) license

• In good standing with all licensing boards and regulatory agencies

SNF or long-term care experience preferred

• Familiarity with Net Health documentation system a plus

• Strong communication and clinical skills
