The North County Role
As a vital member of our interdisciplinary team, you will travel to patient homes in the Oceanside, Escondido, Vista, San Marcos, and Carlsbad regions to deliver personalized physical therapy services.

Setting: Home Health

Territory: Oceanside, Escondido, and surrounding North San Diego County.

Schedule: Per Diem/PRN - Choose your own hours based on patient needs and your availability.

Compensation: Highly competitive per-visit rate.
- Perform comprehensive physical therapy evaluations and develop individualized Plans of Care (POCs) to restore function, reduce pain, and prevent disability.
- Provide skilled treatment including therapeutic exercise, transfer training, gait training, and patient/caregiver education.

Coordinate care effectively with nurses, occupational therapists, speech-language pathologists, and physicians to ensure holistic patient recovery.
- Ensure timely, accurate, and compliant clinical documentation using an Electronic Medical Record (EMR) system.
- Monitor patient progress, communicate changes in condition to the physician, and participate in discharge planning.
- Current and unrestricted Physical Therapist (PT) license in the State of California.

Minimum of 1 year of clinical experience as a PT (Home Health experience is strongly preferred).
- Current CPR certification.
- Doctor of Physical Therapy (DPT) degree is highly desirable.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Strong organizational skills and an independent work ethic.
