Family Nurse Practitioner Integrated Behavioral Health & Primary Care
Job Description
Reset Wellness & Recovery is seeking a compassionate, motivated, and team-oriented Family Nurse Practitioner (FNP) to join our growing integrated outpatient treatment program in Phoenix, Arizona. This role is ideal for a provider who is passionate about whole-person care and comfortable working within an integrated care model that supports both behavioral health and primary care collaboration.
Our program serves individuals with mental health and substance use needs while also addressing overall wellness, medical support, and continuity of care. The ideal candidate is flexible, collaborative, and experienced—or eager to grow—in a setting where psychiatric and primary care services overlap.
Responsibilities
• Conduct comprehensive medical evaluations and follow-up appointments
• Support integrated care coordination between behavioral health and primary care teams
• Collaborate with psychiatric providers, therapists, case managers, and nursing staff
• Assist with medication management and monitoring within scope of practice
• Address common primary care concerns and chronic health conditions
• Support continuity of care for individuals with co-occurring disorders
• Complete timely and accurate documentation within EHR systems
• Participate in treatment planning and interdisciplinary team meetings
• Promote patient-centered, trauma-informed, and culturally responsive care
Qualifications
• Active Arizona FNP license in good standing
• DEA license preferred or ability to obtain
• Experience in behavioral health, substance use treatment, primary care, or integrated care preferred
• Comfortable working with underserved and high-acuity populations
• Strong communication and collaboration skills
• Ability to work independently while functioning within a multidisciplinary team
• Experience with outpatient settings and EHR documentation preferred

• MAT/Substance use treatment exposure
• Mental health medication familiarity
• Dual diagnosis/co-occurring disorder populations
• Community health or outpatient treatment environments
