Nurse Practitioner

We are seeking a Nurse Practitioner to become a part of our team! You will focus on providing high quality patient care as part of a healthcare team.

PART TIME / FULL TIME

Responsibilities:

• Evaluate and manage patients in an outpatient primary care setting
• Diagnose and treat common acute and chronic conditions
• Perform annual physical exams and preventive care visits
• Manage chronic diseases (HTN, diabetes, hyperlipidemia, etc.)
• Order and interpret labs and imaging
• Prescribe medications according to NY state regulations
• Provide patient education and counseling
• Document encounters in eClinicalWorks
• Collaborate with supervising physician and clinical staff

Qualifications:

• Active New York State Nurse Practitioner license
• Board certification (ANCC or AANP)
• Minimum 2 years primary care experience preferred
• Strong clinical and diagnostic skills
• Comfortable seeing 20-30 patients per day
• Experience with EHR systems (eClinicalWorks preferred)
• Excellent communication and patient care skills
• Bilingual English/Spanish preferred
