Positive Growth, Inc. is seeking a qualified Human Services Professional (HSP) to support youth in our residential program.

The HSP is responsible for coordinating and documenting services that support each resident’s safety, permanency, well-being, and individualized goals. This position works closely with residents, families, DFCS case managers, schools, healthcare providers, therapists, residential staff, and other members of the treatment team.

Key Responsibilities
  • Complete required individual contacts with assigned residents.
  • Coordinate medical, dental, behavioral-health, educational, and other needed services.
  • Participate in service planning, treatment-team meetings, and permanency activities.
  • Maintain communication with DFCS, families, schools, and community providers.
  • Monitor resident progress and follow up on identified needs.
  • Maintain accurate and timely case documentation, contact notes, assessments, reports, and service records.
  • Track required appointments, records, referrals, and deadlines through completion.
  • Support educational advocacy, family engagement, independent-living skills, and discharge planning.
  • Identify and report safety concerns, emerging risks, or significant changes in resident functioning.
  • Correct documentation or compliance deficiencies when identified.
  • Work collaboratively with the Quality Assurance staff.
Qualifications
  • Must meet applicable qualification requirements to serve as an approved Human Services Professional within a Georgia residential child-care program.
  • Bachelor’s degree in an applicable human-services-related field.
  • Relevant experience in child welfare, residential care, social services, behavioral health, case management, or youth services preferred.
  • Strong written documentation, communication, organizational, and follow-through skills.
  • Ability to manage multiple residents, deadlines, appointments, and documentation requirements.
  • Must meet all agency background, training, and employment requirements.
Preferred Candidate

We are seeking an individual who is dependable, organized, and professional, and who can work independently while remaining part of a multidisciplinary team. The ideal candidate understands that successful case coordination requires consistent follow-up and completion.

Experience working with adolescents in residential care, foster care, behavioral health, or other child-serving systems is strongly preferred.
