Salary: Up to $130,000 per year

The Emerald Peek Rehabilitation and Nursing Center is seeking an experienced and detail-oriented MDS Coordinator (RN) to join our clinical leadership team.

This is an excellent opportunity for a Registered Nurse with strong MDS, clinical documentation, and reimbursement experience in a skilled nursing or long-term care setting. The MDS Coordinator will play a key role in ensuring accurate assessments, regulatory compliance, and optimal reimbursement while supporting exceptional resident care.

Responsibilities
  • Coordinate and complete MDS assessments accurately and within required timeframes.

  • Manage the full MDS process, including scheduling, assessments, care area triggers, and documentation.

  • Ensure compliance with CMS, state, and federal regulations.

  • Review clinical documentation to ensure accuracy and support appropriate coding and reimbursement.

  • Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.

  • Participate in care plan meetings and ensure resident assessments reflect current needs.

  • Monitor MDS schedules, submission deadlines, and validation reports.

  • Identify documentation and coding opportunities and provide education to clinical staff as needed.

  • Assist with Medicare, Medicaid, and other reimbursement-related processes.

  • Stay current on changes to MDS requirements, RAI guidelines, PDPM, and applicable regulations.

  • Participate in quality improvement and survey readiness initiatives.

  • Maintain accurate and organized records related to MDS assessments and submissions.

Qualifications
  • Current and active Registered Nurse (RN) license in the applicable state.

  • Previous experience as an MDS Coordinator in a Skilled Nursing Facility or Long-Term Care setting is strongly preferred.

  • Strong knowledge of MDS, RAI, PDPM, Medicare, Medicaid, and reimbursement processes.

  • Experience with MDS software and electronic medical records.

  • Strong clinical assessment and documentation skills.

  • Excellent organizational, analytical, and time-management abilities.

  • Ability to manage multiple deadlines and priorities.

  • Strong communication and collaboration skills.
