About the Role:
As a Registered Nurse MDS Coordinator, you will be responsible for:
Coordination and Management: Oversee the completion of Minimum Data Set (MDS) assessments, ensuring accurate and timely documentation in accordance with regulatory requirements.
Clinical Expertise: Utilize your nursing expertise to develop and implement individualized care plans that meet the unique needs of our residents.
Communication and Collaboration: Work closely with interdisciplinary teams, physicians, and other healthcare professionals to ensure seamless care delivery and optimal resident outcomes.
Leadership and Guidance: Mentor and train staff members on MDS assessment and care planning best practices.
Quality Assurance: Participate in quality improvement initiatives to ensure compliance with regulatory standards and optimization of care processes.
Requirements:
License and Certification: Active RN license in the state of Michigan with current certification in nursing or related field.
