Northbrook Health and Rehab, a leading long-term care and rehabilitation facility, is seeking a compassionate and dedicated Registered Nurse (RN) to join our team. As an RN, you will provide high-quality, patient-centered care to residents in a supportive, team-oriented environment. Our goal is to enhance the quality of life for individuals requiring long-term care, rehabilitation, and support, while maintaining their dignity and comfort.


Key Responsibilities:

  • Direct Patient Care: Monitor and assess patients' health conditions, administer medications, and provide treatments as prescribed by physicians.

  • Care Planning: Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized care plans for residents.

  • Patient Advocacy: Serve as a liaison between residents, families, and healthcare providers, ensuring residents’ needs are met and their concerns are addressed.

  • Documentation: Maintain accurate, up-to-date records of residents’ health status, treatments, and progress.

  • Emergency Response: Respond to emergencies, providing immediate care and coordinating with physicians, specialists, and other healthcare team members.

  • Supervision: Supervise and mentor nursing assistants, LPNs, and other support staff to ensure high standards of care are met.

  • Compliance: Adhere to all healthcare regulations and facility policies, maintaining a safe and effective environment for residents and staff.

Qualifications:

  • Current and valid Registered Nurse (RN) license in Illinois.

  • Strong clinical assessment skills, with the ability to recognize and respond to changes in patient conditions.

  • Excellent communication, organizational, and interpersonal skills.

  • Ability to work effectively in a collaborative, team-based environment.

  • Compassionate, patient-focused, and detail-oriented with a strong sense of integrity and professionalism.

  • CPR and BLS certification.
