Gulfside Health & Rehab is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our team in Clearwater, FL. We are actively hiring and looking to fill this role immediately.

• Complete and oversee MDS assessments and care planning in accordance with federal and state regulations

• Ensure accuracy and timeliness of MDS submissions

• Collaborate with interdisciplinary teams to optimize reimbursement and quality outcomes

• Monitor case mix index (CMI) and identify opportunities for improvement

• Maintain compliance with Medicare/Medicaid guidelines and facility policies

• Participate in audits and support survey readiness

• Active RN license in the state of Florida (required)

• 1+ years of MDS experience in a skilled nursing facility (required)

• Strong knowledge of RAI process, PDPM, and Medicare guidelines

• Detail-oriented with strong organizational and communication skills

• Ability to work independently and as part of a collaborative team

• $80,000–$100,000 per year based on experience
