Job Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) Concierge Nurse to join our dedicated team at Highfield Gardens Care Center of Great Neck, a sub-acute and long-term care facility in Great Neck, New York. As a RN Concierge Nurse for Weekends, you will have the opportunity to provide exceptional care and support to our residents during the weekends, ensuring they receive the highest quality care and attention.
RN Concierge Nurse Responsibilities:
Provide direct patient care to residents, including administering medications, performing treatments, and monitoring vital signs
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Conduct assessments, evaluations, and document patient progress
Ensure a safe and clean environment for residents by following infection control procedures and maintaining a tidy living space
Provide emotional support and companionship to residents, promoting a sense of community and well-being
Participate in quality improvement initiatives to improve patient outcomes and care delivery
Maintain accurate and detailed records of patient care and treatment plans

Requirements:
Current RN license in the state of New York
Prior experience in long-term care or sub-acute care preferred (new graduates welcome!)
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Familiarity with electronic medical records (EMRs) and ability to adapt to our system
