Title: Counselor – Center of Recovery (CORE)


Function:


The Counselor acts as case manager/counselor for assigned clients.

Supervisor:


CORE Director of Clinical Services and/or designee.

General Work Schedule:


Monday – Friday 8:00 AM – 5:00 PM

Qualifications:


1. Master’s degree in counseling or related field of study.
2. CCGC / LPC / LCSW status current or in process of completion.
3. Pre-Employment negative drug screen and background check.
4. Free from TB and verified by annual skin test by CORE.

Duties and Responsibilities:


1. Conducts in-patient process groups, lectures, individual and family sessions with clients.

2. Prepare educational, group and individual progress notes on each client within 24 hours and 48 if PT (if needed)

3. Prepares and completes discharge summaries on assigned clients within 72 hours

4. Works with assigned clients to formulate a Health & Recovery Plan and coordinate continued support services such as a counselor, GA meetings, sponsor, etc.

5. Receives intake calls in the absence of the Intake Specialist, fills out necessary paperwork, and notifies Intake Specialist of the intake

6. Performs intake interviews and client assessments to determine the needs and severity of the gambling addiction for each potential client if the Intake Specialist is unavailable.

7. Meets individually with each assigned client to go over treatment plan and objectives

8. Completes the assessment on assigned clients within 72 hours of admission.

9. Coordinates client information on a daily and weekly basis with the rest of the clinical staff

10. Coordinates with the medical doctor and contract RN on patient care issues.

11. If necessary, assists with pick up of client medication as authorized by the contract nurse or director

12. If necessary, assist assigned clients with medical appointments and finding housing after discharge, if required.

13. If necessary, assist in supervision of intern counselors by co-facilitating/shadowing in group, assigning client tasks such as initial assessments, etc.

14. Assists the rest of the clinical staff in planning and formulating treatment goals and objectives for the overall therapy program.

15. Provides outreach lectures when requested by Director of Clinical Services.

16. Maintains assigned patient records and ensures they meet clinical guidelines

17. Supervises the updating and creation of educational, step, and relapse prevention materials for use in treatment

18. Supervises and documents all medications taken by clients

19. Is authorized to call, charge, and schedule bus transportation for clients for admit and/or discharge

20. Attend staff and clinical meetings

21. Other duties as assigned by Director of Clinical Services and/or designee.
