Your Impact (Key Responsibilities)
  • Perform comprehensive wound and skin assessments

  • Develop, implement, and manage individualized wound care plans

  • Administer treatments and ensure accurate, timely documentation

  • Monitor healing progress and adjust care plans with physicians

  • Track and report pressure ulcers and skin integrity data

  • Lead prevention programs (Braden Scale, risk interventions)

  • Educate and mentor nursing staff on best practices in wound care

  • Ensure compliance with state, federal, and facility standards

  • Support Quality Assurance & Performance Improvement (QAPI) initiatives

  • Maintain proper wound care supplies and resources

What We’re Looking For
  • Active RN or LPN/LVN license (Missouri)

  • At least 1 year of wound care or treatment experience in long-term care

  • Preferred: 2+ years in Skilled Nursing Facility (SNF)

  • Current BLS/CPR certification

  • Strong clinical judgment and documentation skills

  • Passion for resident-centered care and outcomes
