What You'll Do:

• Support a dynamic retinal practice with in-office patient care
• Connect with patients monthly through CCM and BHI outreach calls
• Help chronic and behavioral health patients stay on track between visits
• Coordinate care, monitor progress, and be a trusted voice patients look forward to hearing

What We're Looking For:

• Certified Medical Assistant (CMA) preferred
• Strong communication skills and a warm, patient-centered approach
• Self-motivated and able to manage your own patient caseload
• Ophthalmology or specialty experience a plus
• CCM/BHI experience a plus but we will train the right person!
