About the Role:
We are seeking an experienced Survey Readiness & Quality Assurance Nurse to support two Assisted Living communities in Ohio. This role is responsible for ensuring continuous compliance with Ohio Department of Health (ODH) regulations, maintaining survey readiness, and leading quality improvement initiatives across both locations.
Key Responsibilities:
Float between Century Farms Assisted Living and St. Clair Commons Assisted Living based on facility needs and survey priorities.
Maintain ongoing survey readiness in accordance with Ohio Department of Health (ODH) regulations.
Conduct routine and focused audits of resident care plans, medication administration, clinical documentation, infection control practices, and resident rights compliance.
Lead and prepare facilities for state surveys, including mock surveys and readiness assessments.
Develop, write, and oversee Plans of Correction (POCs) for Ohio Department of Health citations.
Monitor corrective actions to ensure sustained compliance.
Partner with Administrators, Wellness Directors, and leadership to identify risk areas and implement corrective strategies.
Review incidents, trends, and QA data to prevent recurrence and improve outcomes.
Provide staff education related to ODH regulations, documentation standards, infection prevention, best clinical practices, and survey preparation.
Qualifications:
Active Ohio RN or LPN license in good standing (required)
Minimum 3 years of experience in Assisted Living, Long-Term Care, or similar healthcare setting
Strong background in survey preparation, quality assurance, compliance, and regulatory oversight
Demonstrated knowledge of Ohio Department of Health (ODH) regulations
Experience developing and managing Plans of Correction
Excellent documentation, organization, and communication skills
Ability to work independently and travel between facilities as needed
Proficiency with electronic medical records (PointClickCare preferred)
High level of professionalism, accountability, and attention to detail
