Position Overview

Neighborhood Community Services (NCS) is seeking a licensed mental health professional to provide high-quality, community-based clinical services to individuals and families. This clinician's role focuses on direct service delivery, clinical documentation, and collaboration with treatment teams to ensure effective and ethical care.


The ideal candidate is compassionate, organized, and experienced in providing in-home or community-based mental health services.


Key Responsibilities

  • Provide direct oversight to community-based clinical services
  • Supervise QMHP and QMHP-T's
  • Conduct consumer assessments, re-assessments, and admissions
  • Ensure compliance in the development, implementation, and modification of individualized treatment plans, and quarterly reviews
  • Maintain accurate, timely, and compliant clinical documentation
  • Coordinate and participate in treatment team meetings and collaborate with interdisciplinary staff
  • Contribute to discharge planning and continuity of care
  • Ensure services meet agency standards, state regulations, and clinical best practices
  • Submit required administrative and clinical reports, including weekly reports due Mondays by 5:00 PM


Qualifications

  • Active, unrestricted clinical license in the state of practice (LCSW, LPC, LMFT, LMHC, or equivalent)
  • Master’s degree in Social Work, Counseling, Psychology, or a related field
  • Experience providing community-based and/or in-home mental health services preferred
  • Strong assessment, treatment planning, and documentation skills
  • Ability to work independently while collaborating with a clinical team
  • Reliable transportation for in-home services, as applicable
