Family Care of Hartford Corp is looking for a passionate and dedicated Family Nurse Practitioner to join our growing healthcare team in Hartford, CT. This is an exciting opportunity to make a meaningful impact in the lives of patients across all ages while working in a collaborative and supportive clinical environment.
Responsibilities:
- Provide comprehensive primary care services to patients of all ages, from pediatrics to geriatrics
- Conduct thorough patient assessments, physical exams, and health screenings
- Diagnose and treat acute and chronic illnesses, injuries, and conditions
- Develop and manage individualized patient care plans and treatment protocols
- Prescribe medications and therapies in accordance with state regulations and clinical guidelines
- Educate patients and families on preventive care, wellness, and disease management
- Maintain accurate and up-to-date electronic health records (EHR) documentation
- Collaborate with physicians, specialists, and care teams to ensure continuity of care
- Active Family Nurse Practitioner (FNP) certification from AANP or ANCC
- Current Connecticut Advanced Practice Registered Nurse (APRN) license
- Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
- DEA license and prescriptive authority in the state of Connecticut
- 3-5 years Experience in primary care, family medicine, or outpatient clinical setting preferred
- Proficiency with electronic health records (EHR) systems
- Strong interpersonal and communication skills with a patient-centered approach
- Ability to work independently and as part of a multidisciplinary healthcare team
