Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at Aperion Care Wesley in Chicago, Illinois. As a Registered Nurse with our organization, you will have the opportunity to provide high-quality patient care while working in a dynamic and supportive environment. This is a full-time position with a Both schedule.
Responsibilities
Provide direct patient care to residents with various medical conditions, including post-hospital rehabilitation and long-term skilled nursing care
Develop and implement individualized care plans to meet the unique needs of each resident
Collaborate with other healthcare professionals to ensure seamless communication and coordination of care
Monitor and report changes in resident condition to the medical director and other healthcare professionals
Document resident care and progress in accordance with regulatory requirements and organizational policies
Participate in quality improvement initiatives to promote excellence in patient care
Other duties as assigned
