About the Role:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Per Diem team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin. As a Registered Nurse, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and maintaining a safe and supportive environment for our residents. Responsibilities:
Provide direct patient care to our residents, including assessments, interventions, and evaluations
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor and report changes in patient condition to the healthcare team
Maintain accurate and up-to-date patient records
Participate in quality improvement initiatives to enhance patient outcomes
Collaborate with other healthcare professionals to deliver comprehensive care
Requirements:

As a Registered Nurse (RN) at Rock River Nursing & Rehab, you must:
Possess a current RN license in the state of Wisconsin
Have a minimum of 1+ years of experience in a long-term care or acute care setting
Demonstrate excellent communication, problem-solving, and critical thinking skills
