Job Description
The Community Placement Case Manager (Community Placement Services) helps children and youth resolve behavioral, mental, and emotional issues. This often involves helping them deal with trauma and feelings of abandonment. Case Managers meet with youth and foster families on a regular basis. This position will identify the needs of children and youth in LUK foster homes and connect them to community resources to meet those needs. This position will gather information via screenings, assessments, and consultation with collateral sources. With the youth, the Case Manager will develop goals for an Individualized Action Plan. Ratings on progress toward these goals as well as discharge planning and implementation are expected. The Case Manager will work with other members of the youth and foster family’s team to ensure children and youth in foster care receive the support needed to thrive.
The qualified candidate will have a Bachelor’s or higher degree in Human Services, Counseling, Social Work or closely related field. Excellent communication and computer skills required. Credentials as a Licensed Social Work Associate (LSWA), Licensed Social Worker (LSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) by the Massachusetts Board of Registration in Social Work preferred.
This is a fulltime position located in North Central Massachusetts and will include community-based travel and implementation. You must have reliable and safe transportation. Work schedule must accommodate the needs of the youth/foster family. Bilingual/Bicultural candidates are encouraged to apply.

The Community Placement Case Manager will provide strength-based services to children and youth to support progress toward their goals.
- Meet with children and youth at their foster home, school, or other community-based location regularly
- Maintain communication with youth, foster families, and other treatment team members between meetings
- Complete intakes and discharges in a timely manner, to include screening and assessments as well as individual action plan development
- Identify children and youth’s needs and connect them to community resources to meet those needs (warm hand-off preferred)
- Complete regular reviews of progress toward goals to quantify child/youth functioning
- Model skills for youth and coach/mentor them to develop their own skills
- Assist with internal and external reports
- Actively participate in supervision and training as required
- Ensure safety and well-being of all youth by observing for signs of abuse and/or neglect
- Provide services that respect each family and youth’s cultural background, heritage, and identity
- Participate in emergency services rotation
- Gain an understanding and commitment to LUK’s Mission, Vision, and Values and provide congruent clinical services
Knowledge and Experience Required:
- Bachelor’s Degree in Human Services, Counseling, Social Work, Psychology or closely related field
- 1 year of experience providing case management
- Bilingual/bicultural candidates encouraged to apply
