A Moment’s Notice Health Care (AMNHC) is a locally managed home health care agency. We service high-end clients in Indian River and the surrounding counties. We are seeking caring, dependable, and team -oriented individuals who possess a true passion for caring for our geriatric community. This position is per diem and offers an extremely flexible schedule. AMNHC is continuing to grow at a fast pace, and this position could, over time, become full-time if the applicant is interested.

We are currently hiring an Intake Specialist with two (2) or more years' experience in private duty home care. As an Intake Specialist, you will meet with clients and their families to conduct client intake assessments, including detailed histories, prepare care plans providing effective and efficient medical care in accordance with AMNHC policies and federal/state laws/regulations.

Qualifications:

  • RN / LPN preferred

  • At least two years of private home care experience.

  • Valid FL Driver's License

  • Vehicle that is currently insured.

  • Cell phone with Internet and texting capabilities.

  • Daily access to AMNHC’s email.

  • Must complete all physical and mental demands, which may include, but not be limited to the following: the ability to transfer clients, carry supplies, stand, walk, use hands, reach, stoop, kneel, crouch, and speak effectively.

  • CPR/BLS Certification

  • Work with pets when necessary

Responsibilities:

Primarily: Perform initial client assessments

  • On occasion, meet with families who are not ready to commit to AMNHC but want more information on our services.

  • As bring on clients:

    - Developing the care plan and ENSURING IT IS COMMUNICATED AND UNDERSTOOD BY CAREGIVERS

- Monitoring task lists and determining appropriate reporting/charting

- Responding promptly to calls, texts and emails from the care team and family members – including during after hours

  • Appropriate response times to questions and issues

  • Ongoing and regular communication with the Administrator

  • Respectful and constructive interaction with all areas of AMNHC operations and its team members

  • Complies with policies and procedures including standards of conduct and work ethics, HIPAA policy, dress code and weekend / holiday availability policies.

  • Other functions as needed

  • Travel within Indian River County & St Lucie County

Language: English, Spanish preferred
