SUMMARY. The Client Case (CC) Manager has oversight of the youth client's safety, care, social & service support and development. The principal role of the CC Manager is for social & service support. The CC Manager is the client's primary advocate and liaison. The CC Manager is the contact person for the child placement agency's case managers, therapists, schools, employees, other outside agencies or family members. The CC Manager assists with entry of the client to the facility, initial assessment, regular reporting, all health-related appointments and a monthly program plan and any other client related social and service support. ESSENTIAL FUNCTIONS:Supervised by the Director The Client Case Manager is a role model in behavior and work ethic for the staff and clientsAdheres to Avery's Village, LLC. Employee Policies and Procedures manualCompletes all required trainingManages and responsible for a case load for up to 16 youth clientsLiaison with the Child Placement Agencies (KVC, St. Frances, TFI and Cornerstones of Care) for placing clients at Avery's Village and all other client needsEnsures all documentation for clients are received for placementCoordinates the admission of all new clientsConducts an initial assessment of new clientsCreates and maintains the case books for clientsRequests the 30-day Qualified Residential Treatment ProgramEnsures clients have completed eye, dental and physical health examinationsSets up medication and/or therapy appointmentsWrites weekly and monthly progress reports on the clientsCreates, monitors and updates a program treatment plan for each clientCreates and implements a transition plan and discharge documentation for all clientsProvide crisis intervention for clientsMake and assists with transportation of clients to appointmentsAssist clients with school and employmentLiaison for communication, facilitation or meetings with CPA's case managers, therapists, schools, employees, other outside agencies, foster families or family memberAssist Client Care Tech on the floor with clients if neededPerforms other duties assigned by the Director PHYSICAL DEMANDS:Frequently move/lift between 25-50 pounds, depending on assignment requirementsPerform all duties that may require sitting, kneeling, squatting and walkingManual dexterity and fine motor coordination REQUIRED EDUCATION AND EXPERIENCE:Candidates must be at least 21 years of age or olderBachelor's degree from an accredited college in one of the Human Services related fields2-5 years previous experience as a case managerFamiliarity with mental health theory and practiceProficient technology skills utilizing Windows platform, including the Microsoft Office suite (Word, Excel, Outlook, PowerPoint) required DESIRABLE KNOWLEDGE, SKILLS, and ABILITIES: Current CPR/First Aid CertificationKnowledgeable and sensitive in working with culturally diverse staff and clientsSolid knowledge in social helping and a positive approach to complex mental health issuesExcellent interpersonal and communication skills, including both oral and written.Must have strong skills in organization and planning, demonstrated ability to work independently and problem solveDemonstrated ability to manage multiple people and projectsAbility to prioritize tasks, exercise sound judgment and confidentiality with sensitive informationAbility to establish effective working relationships with staff, community agencies, and consumers OTHER POSITION ELIGIBILITY REQUIREMENTS:If required to operate a company vehicle during the course of employment, must possess a current state driver's license and have an acceptable driving record for the past three (3) yearsAbility to pass a criminal background clearance check and a physical exam and TB test.
