Prestige Healthcare Resources is seeking a fully licensed clinician (LICSW or LPC) to join our Admissions Department as a Clinical Care Coordinator. This role is essential as MHRS continues to evolve, and we are expanding our point-of-service clinical support for consumers who do not yet have authorizations.

Position Overview

The Licensed Clinician will provide clinical care coordination for consumers enrolled in the MHRS, behavioral health program. This role focuses on point-of-service care, ensuring consumers receive immediate clinical support.

Key Responsibilities

  • Provide point-of-service clinical care coordination for consumers.
  • Conduct crisis assessments and interventions as assigned.
  • Maintain accurate, timely clinical documentation.
  • Collaborate closely with Admissions, MHRS, and program leadership.
  • Participate in Saturday rotation as scheduled.
  • Uphold all DBH, Medicaid, and MHRS compliance standards.

• $70 per billable hour

Department: Admissions Primary Function: Point-of-Service Clinical Care Coordination for consumers without authorizations; support for authorized consumers; crisis support; DA20s; treatment plan updates.

1. Care Coordination Productivity

  • 15–20 care-coordination encounters per week (mix of point-of-service, follow-ups, and coordination tasks).
  • 90% of consumers without authorizations must be seen within 48 hours of referral.
  • Documentation completed within 24 hours of each encounter.

2. Authorization & Coverage Support

  • 100% verification of consumer authorization status before care coordination.
  • All consumers without authorizations must receive a point-of-service care plan at time of contact.

3. Crisis Response

  • Respond to all Admissions-assigned crisis cases within 1 hour.
  • Complete crisis documentation and safety planning same day.

4. Saturday Rotation

  • Maintain 100% attendance for scheduled Saturday rotation shifts.
  • Complete all weekend care-coordination tasks by end of shift.

5. Interdepartmental Collaboration

  • Attend all Admissions meetings and required clinical huddles.
  • Maintain weekly communication with MHRS leadership to stay aligned with evolving requirements.
  • Zero missed handoffs to MHRS,

6. Consumer Outcomes & Engagement

  • 80% of consumers without authorizations must receive a documented care-coordination follow-up within 7 days.
  • Reduction in “no-authorization delays” by 30% within first 90 days of role implementation.
  • Consumer satisfaction score of 4.5/5 (based on brief post-encounter survey).

8. Billing & Compliance

  • Maintain $70 per billable hour productivity target.
  • Zero compliance flags related to documentation, timeliness, or clinical standards.
  • 100% adherence to Medicaid, DBH, and MHRS documentation requirements
