What You'll Do

• Partner with insurance companies and payers to advocate for appropriate rehabilitation and medical services
• Conduct comprehensive assessments of patients' medical, social, and psychosocial needs
• Collaborate with interdisciplinary team members to develop individualized care plans
• Coordinate and monitor rehabilitation, medical, and support services
• Advocate for patients and ensure their needs are addressed throughout the recovery process
• Provide education, resources, and support to patients and their families
• Facilitate communication between patients, families, providers, and community resources

Responsibilities

What you will do: Responsibilities listed include but not limited to:

  • Work with insurance and payers to advocate for the appropriate rehabilitation and medical care.
  • Conduct comprehensive assessments of patients' medical, social, and psychological needs
  • Collaborate with the treatment team to develop individualized care plans.
  • Coordinate and monitor the delivery of services, including therapy, medical care, and support services
  • Advocate for patients' rights and ensure their needs are met throughout their rehabilitation journey
  • Provide education and support to patients and their families

Qualifications

What you will need:

  • Bachelor's or Master's degree in a related field preferred (e.g., Social Work, Psychology)
  • Clinical case management experience, preferably in a rehabilitation or healthcare setting
  • Strong interpersonal and communication skills
  • Knowledge of brain injuries and rehabilitation preferred
