Job Title: Registered Nurse – Clinical Manager (Home Health)

Job Summary
We are seeking an experienced and detail-oriented Registered Nurse (RN) to join our team as a Clinical Manager for Home Health. The ideal candidate will have a strong clinical background in home health, excellent leadership skills, and hands-on experience with EMR systems, preferably Kinnser (WellSky). This role is responsible for overseeing clinical operations, ensuring compliance, and maintaining high-quality patient care.

Key Responsibilities


  • Oversee day-to-day clinical operations of home health services
  • Supervise and support field clinicians including RNs, LVNs, PTs, OTs, and aides
  • Review, approve, and ensure accuracy of OASIS documentation and care plans
  • Ensure compliance with state, federal, and accreditation standards
  • Coordinate patient care services and ensure timely admissions, recertifications, and discharges
  • Monitor clinical outcomes and implement quality improvement initiatives
  • Provide clinical guidance and training to staff
  • Collaborate with physicians, referral sources, and interdisciplinary teams
  • Ensure proper utilization of EMR systems for documentation and workflow efficiency
  • Participate in audits and ensure readiness for surveys
Qualifications
  • Active Registered Nurse (RN) license in [State]
  • Minimum 3+ years of home health experience (mandatory)
  • Prior experience in a Clinical Manager or supervisory role preferred
  • Strong knowledge of Medicare guidelines, OASIS, and home health regulations
  • Experience with EMR systems required; Kinnser (WellSky) preferred
  • Excellent organizational, communication, and leadership skills
