Responsibilities:

• Develop and implement personalized care plans for clients, taking into account their medical, physical, and emotional needs

• Coordinate and supervise care services, including home health, hospice, and palliative care, to ensure seamless transitions and effective care delivery

• Collaborate with clients, family members, healthcare providers, and other stakeholders to ensure that care goals are met and client needs are met

• Conduct assessments, evaluations, and progress reports to ensure optimal client outcomes and quality of care

• Manage and develop staff, providing guidance and support to ensure effective care delivery and staff development

• Maintain accurate records and reports, adhering to regulatory requirements and company policies

Requirements:

• 3-5 years of experience in care management, healthcare, or a related field

• Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple projects simultaneously

• Excellent communication, interpersonal, and organizational skills, with the ability to work effectively with diverse stakeholders

• Ability to work in a fast-paced environment, with flexibility to accommodate changing priorities and schedules

• Strong attention to detail, with the ability to maintain accurate records and reports
