Everbloom Memory Care is seeking a strong, experienced and highly proactive Registered Nurse (RN) toserve as our Director of Clinical Services. We currently serve approximately 22 residents and are growingtoward 30 residents. Our community has a strong full-time Administrator overseeing facility operations. Weare seeking an RN who can partner with the Administrator and take independent ownership of the clinicaldepartment. This is a hands-on clinical leadership role for an RN who is organized, responsive, comfortablemaking decisions and able to keep clinical responsibilities moving forward without constant direction fromownership.Clinical Department Leadership Provide leadership and oversight of resident clinical care. Maintain organized, compliant and survey-ready clinical systems. Monitor residents for changes in condition and ensure timely intervention and follow-up. Coordinate with physicians, pharmacies, hospice, home health and other healthcare providers. Ensure orders, documentation, assessments and clinical follow-ups are accurate and timely. Identify clinical risks and proactively implement solutions. Participate in quality assurance and compliance activities. Serve as the primary clinical resource for the Administrator and care team. Take ownership of clinical deadlines and responsibilities without requiring routine follow-up fromownership.Assessments & AdmissionsThe Director of Clinical Services is expected to independently own the clinical assessment and admissionprocess rather than routinely relying on ownership to perform assessments. Independently complete prospective resident clinical assessments. Review medical records, medications, diagnoses, behaviors and care requirements. Determine clinical appropriateness for admission within facility admission criteria. Communicate directly with hospitals, discharge planners, families, placement agencies and healthcareproviders. Respond promptly to new referral and assessment opportunities. Partner with the Administrator in admission decisions. Coordinate clinical preparation for new residents. Ensure medications, orders, care needs and staff education are appropriately addressed for move-in. Complete appropriate post-admission clinical follow-up.Med Tech Training & Clinical Education Lead training and development of medication technicians. Maintain required competencies and continued education. Conduct medication-system, MAR and documentation audits. Identify medication errors, determine contributing factors and implement corrective education. Ensure med techs understand appropriate escalation and change-of-condition procedures. Provide ongoing clinical education to caregivers and medication staff. Develop a confident and accountable medication team.Clinical Oversight & Staff Development Identify staff training needs and implement education. Support caregivers and med techs with challenging resident situations. Assist with clinical orientation of new employees. Reinforce documentation and resident-care expectations. Hold clinical staff accountable while maintaining a supportive team environment. Help develop systems that improve resident outcomes and reduce preventable clinical issues.On-Call & Scheduling Support Participate in clinical on-call responsibilities. Provide appropriate clinical guidance after hours when needed. Partner with the Administrator regarding clinical staffing needs. Assist in ensuring adequate clinical coverage. Develop staff's ability to manage routine situations appropriately and recognize when RN/providerintervention is necessary.Admissions, Marketing & Census Growth Partner with the Administrator to support appropriate census growth from approximately 22 toward 30residents. Develop strong relationships with hospitals, discharge planners, placement agencies, hospice, homehealth and other referral partners. Participate in tours and family meetings when clinical expertise is beneficial. Help families understand how Everbloom can safely meet their loved one's needs. Participate in appropriate networking and community outreach. Represent Everbloom professionally in the healthcare community. Help make the referral-to-admission process responsive, professional and efficient.Qualifications Current Oregon RN license in good standing. Memory Care, Assisted Living, RCF, long-term care, hospice, home health or similar experiencepreferred. Clinical leadership experience strongly preferred. Strong resident-assessment skills. Strong medication-management knowledge. Experience educating or supervising clinical/care staff preferred. Excellent organization and time-management abilities. Strong clinical judgment. Excellent communication with families and healthcare professionals. Ability to work independently and take initiative. Dependable, accountable and solution-oriented.Compensation$90,000-$105,000/year + $500 per successful admissionPLUS $500 bonus for each successful resident admission.Who Will Be Successful Here?We are looking for an RN who wants to lead, not simply complete nursing tasks.Our ideal RN sees what needs attention and takes responsibility for getting it completed.
