Social Worker for Adult Day Care -P/T
As a social worker for an adult day care center creates and implements individualized service plans for participants, provides social and emotional support, and acts as a liaison with families and community resources. Their duties include assessing client needs, developing care plans, coordinating services, and advocating for clients and their caregivers. This role is vital for ensuring the overall well-being of participants in a social adult day care setting.
Key responsibilities
· Client assessments: Meet with individuals and families to assess their needs and create support plans.
· Care plan development: Develop and maintain individualized care plans for each participant, often in an interdisciplinary setting.
· Direct support: Provide emotional and practical support to clients, helping them build trust and confidence.
· Service coordination: Coordinate with community resources and providers to ensure continuity of care.
· Family support: Advocate for the needs of both the participants and their family caregivers.
· Daily activities: Support and participate in daily activities to ensure participants' physical and emotional well-being
Qualifications and skills
· A social work qualification is required, and depending on the role, this could be a Bachelor's, Master's (MSW), or other relevant degree.
· Licensure (e.g., LCSW, LMSW) is often required.
