About the Role:
As a Bilingual Community Outreach Representative at Personal Touch Home Care of N.Y. Inc, you will play a crucial role in connecting with our diverse community, promoting our services, and building relationships with patients and families. Responsibilities:
Conduct community outreach and education programs to promote our services to patients, families, and healthcare providers
Build and maintain relationships with community organizations, health centers, and social services agencies to identify potential partnerships and referrals
Identify and address the social, emotional, and physical needs of patients and families, and provide personalized support and resources
Develop and maintain a strong understanding of our services, including home health and home care programs, to effectively communicate our offerings to the community
Collaborate with our multidisciplinary team to develop individualized care plans that meet the unique needs of each patient
Conduct language-specific outreach and education programs to ensure that our bilingual patients and families receive the care and support they need
Requirements:
Strong communication and interpersonal skills, with the ability to effectively communicate with patients, families, and community organizations
Bilingual in English/Spanish and strong reading, writing, and speaking skills
Ability to work independently and as part of a team to achieve goals and objectives
Strong problem-solving and organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously
