Compensation:

$53 - $56 hourly


Responsibilities:
  • Coordinate and manage comprehensive care plans for hospice patients, ensuring they receive personalized and compassionate support.
  • Collaborate closely with our multidisciplinary team to deliver holistic care that addresses the physical, emotional, and spiritual needs of patients and their families.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as necessary to maintain optimal comfort and quality of life.
  • Serve as a trusted advisor to families, providing guidance and education on hospice care processes and helping them navigate challenging decisions.
  • Administer medications and treatments as prescribed, ensuring adherence to best practices and patient safety protocols.
  • Document patient interactions and care activities accurately and promptly in our electronic health records system.
  • Advocate for patients' needs and preferences, ensuring their voices are heard and respected in all aspects of care planning and delivery.

Qualifications:
  • Experience in hospice or palliative care, providing compassionate and empathetic support to patients and families.
  • Ability to develop and manage comprehensive care plans tailored to individual patient needs, ensuring holistic care.
  • Proven track record of effective collaboration with interdisciplinary teams to deliver patient-centered care.
  • Strong communication skills to educate patients and families about the hospice process, fostering understanding and comfort.
  • Ability to conduct thorough home visits, assessing patient conditions and making timely care plan adjustments.
  • Proficiency in symptom management to ensure patients experience the highest quality of life possible.
  • Experience in maintaining accurate and thorough documentation of patient progress and care activities for seamless team communication.
