Blossom Health Care Center, a warm, home-like skilled nursing facility nestled in a quiet residential neighborhood on the Brighton border of Rochester, NY, is seeking an experienced MDS Coordinator (MDS Nurse) to join our close-knit clinical team. The MDS Coordinator position is ideal for a Registered Nurse (RN) who takes pride in accuracy and resident-centered care. Prior experience in a skilled nursing facility is required for the MDS Coordinator role.

MDS Coordinator Duties

• Oversee the Minimum Data Set (MDS) assessment process and ensure timely, accurate completion

• Coordinate resident assessments with the interdisciplinary team

• Manage care planning and ensure compliance with CMS requirements

• Support accurate PDPM reimbursement through proper documentation

• Track assessment schedules and submission deadlines

MDS Coordinator Qualifications
