MDS Coordinator for a 280 Bed Skilled Nursing Facility in Far Rockaway (Queens).
Full Time position, Candidate must have RN License and experience in MDS 3.0 assessments and care planning. Proficiency in Medicare Assessments and Coordination. Knowledge of SigmaCare EMR a plus.
