Linden Place Center for Nursing and Rehabilitation is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our clinical leadership team. This is a full-time opportunity for an RN who is passionate about quality care, accurate clinical documentation, regulatory compliance, and positive resident outcomes.
As the RN MDS Coordinator, you will play a key role in coordinating the Resident Assessment Instrument (RAI) process, developing individualized care plans, supporting accurate reimbursement, and ensuring compliance with federal and state regulations. You will work closely with our interdisciplinary team to ensure residents receive comprehensive, individualized care.

Coordinate and oversee the Resident Assessment Instrument (RAI) process, including MDS assessments, Care Area Assessments (CAAs), and individualized care plans
Ensure MDS assessments are completed accurately and within required regulatory timeframes
Maintain and communicate the MDS assessment schedule for all applicable payer sources
Collaborate with nursing, therapy, dietary, social services, activities, and other interdisciplinary team members
Participate in daily PPS meetings, weekly Medicare reviews, and month-end reimbursement and compliance activities
Gather and validate assessment information through resident interviews, clinical observations, medical record reviews, and collaboration with department leaders
Ensure supporting interdisciplinary documentation is accurate, complete, and timely
Monitor clinical documentation to support quality measures, reimbursement, and regulatory compliance
Assist with medical record audits and quality improvement initiatives
Provide education and guidance to staff regarding MDS documentation, care planning, and related requirements
Stay current with CMS regulations, RAI requirements, PDPM guidelines, and reimbursement practices
Promote resident-centered care and support positive clinical outcomes
Current Registered Nurse (RN) license in the state of North Carolina
Minimum of three (3) years of clinical nursing experience, preferably in skilled nursing or long-term care
Previous MDS Coordinator experience strongly preferred
Current CPR certification
Working knowledge of MDS/RAI processes, Medicare regulations, PDPM, and care planning
Strong organizational, analytical, and time-management skills
Excellent communication and interpersonal skills
Ability to work effectively with residents, families, physicians, and interdisciplinary team members
Strong attention to detail and commitment to accuracy and compliance
