We are seeking a compassionate and dedicated Social Worker to join our interdisciplinary team at our Skilled Nursing Facility. This is an excellent opportunity for a caring professional who is passionate about advocating for residents, supporting families, and helping improve the quality of life for those in our care.

As a Social Worker, you will play a key role in providing emotional support, coordinating care, and ensuring residents receive the resources and services they need throughout their stay.

Responsibilities:
  • Conduct psychosocial assessments upon admission and as needed

  • Develop and implement individualized care plans with the interdisciplinary team

  • Provide counseling and emotional support to residents and their families

  • Coordinate discharge planning and facilitate smooth transitions of care

  • Assist residents and families with community resources and long-term care planning

  • Advocate for resident rights and well-being

  • Participate in care plan meetings and family conferences

  • Maintain accurate documentation in accordance with federal, state, and facility regulations

  • Collaborate with physicians, nursing staff, and other healthcare professionals to ensure the highest quality of care

Qualifications:
  • Bachelor's Degree in Social Work (BSW) required; Master's Degree (MSW) preferred

  • Previous experience in a skilled nursing, rehabilitation, or long-term care setting preferred

  • Knowledge of discharge planning and long-term care regulations

  • Excellent communication, interpersonal, and organizational skills

  • Compassionate, dependable, and resident-focused

  • Ability to work effectively in a fast-paced, team-oriented environment
